Membership

Personalized, relationship-centered care without the rushed visits or complicated billing. Our monthly membership gives you greater access to your physician, transparent pricing, and a primary care experience built around you.

Primary Care Reimagined Membership and Fee Schedule

Initial Enrollment Fee

 

A one-time, non-refundable $80 enrollment fee is charged to each new billing account (individual or family). Children enrolling under a family account are exempt from this fee.

 

Membership Fees

 

Adults:

  • 18-29 $100 Monthly or $1140 Annual

  • 30-44 $150 Monthly or $1710 Annual

  • 45-60 $185 Monthly or $2109 Annual

  • 61 and over $210 Monthly or $2394 Annual

Kids:

  • 10-17 $60 Monthly or $684 Annual

Family Membership: $420 monthly (2 adults <60 with 2 children age 10-17)

Billing and Membership Details

 

Membership dues begin upon enrollment and are paid monthly or annually on an ongoing basis. Payment of membership dues is required to maintain active member status and access healthcare services.

  • Payment Method: All members must be enrolled in Auto Pay using a credit or debit card,ACH bank account, FSA, HSA. Please note that processing fees only apply to credit card transactions. No additional charges are incurred for payments made via debit cards, HSA/FSA accounts, or ACH bank transfers.We will not be issuing invoices.

  • Commitment: Memberships do NOT require long-term contracts or commitments.

Cancellation

​

  • Monthly memberships may be canceled, for any reason, with 30 days notice prior to the next month's automatic payment period.

  • Annual memberships may be canceled with a prorated refund of remaining membership fees calculated at the monthly rate.

  • All charges, including membership fees, will continue to accrue until notice of cancellation is given.

 

Re-Enrollment

​

If a member discontinues their membership, either by choice or due to lack of payment, they may rejoin Primary Care Reimagined in the future. Rejoining will require permission and acceptance by the physician. A re-enrollment fee of $150 per member will be charged to re-establish the membership as active.

 

Services and Fees

 

Comprehensive Access (Included in Membership)

​

  • Visits: All visits with Dr. Schnick at the clinic during regular hours are provided without any additional fees.

  • Communication: Unlimited communications (email, phone, text, patient portal) are included.

  • Preventive Labs: Yearly wellness/physical labs are fully covered.

​

Additional Services and Discounts

​

  • In-House Labs: Select in-house labs such as flu, COVID, strep testing, and urinalysis are provided at a significantly reduced cost.

  • Clinic Prescriptions: For convenience, we offer several prescription medications at wholesale prices when available. Prescriptions can also be sent to the pharmacy as needed or preferred.

  • Radiology Imaging: We use a network of Imaging Centers to help our members save up to 70% off standard insurance pricing.

Specific Service Fees (Not Covered by Membership)

​

  • Orthopedic joint injections: $50 to cover the cost of medication

  • Yearly flu vaccine: $20 (as supplies last)

  • Pap smear for uninsured patients at physician cost
